The state of Indiana and their version of OSHA found the University of Notre Dame did not "maintain safe working conditions" when Declan Sullivan, a 20-year-old junior, died while he was filming football practice from a hydraulic lift in late October when it fell over in winds that exceeded 50 miles-per-hour.
Indiana Department of Labor Commissioner Lori Torres said the school was fined $77,500 for five violations, including failing to properly train the student videographer and failing to have the lift inspected on a regular basis. Torres says
the lift was missing the appropriate warnings and a service manual.
